Contract Length: 13 weeks


Job Description

  • Deliver superior quality pharmaceutical services while adhering to core company values such as respect, teamwork, and resourcefulness.
  • Prioritize patient and employee safety by following safety protocols including safe patient handling, infectious disease control, and workplace hazard reporting.
  • Maintain adequate drug supply and manage procurement processes, including emergency medication orders.
  • Complete annual inventory and manage removal and documentation of outdated or recalled medications.
  • Prepare, sterilize, label, and dispense medications accurately, ensuring competency assessments for high-volume parenteral preparations.
  • Maintain strict control and accountability for all dispensed drugs, including controlled substances.
  • Conduct monthly inspections of pharmacy and patient care areas where medications are stored or administered.
  • Participate in the development and updating of hospital formularies and provide education to professional staff.
  • Provide medication education and counseling to patients, including discharge medication reconciliation.
  • Maintain confidentiality of patient and medical staff information.
  • Participate in quality assurance programs related to drug utilization and effectiveness.
  • Monitor patient drug therapy for contraindications, interactions, allergies, and appropriateness; report adverse drug reactions.
  • Support antimicrobial stewardship efforts and pharmacokinetic dosing management.
  • Function as a role model and resource for pharmacy staff, promoting continuous quality improvement and staff development.
  • Participate in Pharmacy and Therapeutics Committee meetings and implement committee decisions.

Required Qualifications

  • Current State Pharmacist licensure.
  • BLS certification if required by the state.
  • Minimum of 2 years recent hospital pharmacy experience.
  • EPIC electronic medical record experience.
  • Successful completion of all educational requirements mandated for state licensure.

Skills & Clinical Knowledge

  • Medication reconciliation, including discharge medication lists.
  • Communication skills for gathering home medication history and discharge counseling.
  • Antimicrobial stewardship, including interpretation of culture results and knowledge of infections such as UTI, pneumonia, and SSTI.
  • Pharmacokinetic dosing management and monitoring.
  • Anticoagulation dosing management and monitoring.

Additional Information

  • Works under general supervision of the Director.
  • Expected to maintain cleanliness and order in the pharmacy.
  • Participates in new employee training and ongoing education programs.
  • Supports accreditation and regulatory compliance efforts.
