Physical Therapist

Contract Length: 13 weeks

Work Hours:

• Approximately 20 hours per week from 4/21 to 5/8
• 40 hours per week from 5/11 to 6/5

Job Description

• Evaluate assigned patients and develop, implement, and modify treatment plans based on patient needs and physician direction.
• Delegate and direct discipline-specific therapy services for assigned patients.
• Design functional treatment programs to support effective return to community activities.
• Educate patients, families, students, and coworkers.
• Perform professional duties related to patient care, therapy service development, and program delivery.
• Maintain compliance with organizational policies, confidentiality, and professional codes of conduct.
• Promote patient and employee safety, including safe patient handling, materials handling, infection control, and workplace safety.
• Complete comprehensive patient evaluations and develop treatment plans focused on discharge goals.
• Collaborate with interdisciplinary teams to address barriers to discharge, including equipment trials, patient/family training, and planning for continuing services.
• Manage caseload flexibly, including coverage and communication of care plans.
• Document medical records clearly and timely, including daily notes, goal writing, quality assessments, letters of medical necessity, discharge documentation, and charge entry.
• Ensure care is provided under valid physician orders and insurance approvals as required.
• Participate in student education programs with supervision and evaluation responsibilities.
• Develop and pursue individualized professional development plans.
• Engage in patient and professional advocacy activities.
• Adhere to state practice acts, maintain licensure, and complete continuing education.
• Support organizational performance improvement initiatives and provide feedback to maximize outcomes.

Required Qualifications

• Valid, current State Physical Therapist license.
• BLS (Basic Life Support) certification required prior to first shift.
• Minimum of 2 years of experience as a Physical Therapist.
