Travel Speech Language Pathologist (SLP) – Skilled Nursing Facility
Punta Gorda, FL
13-Week Contract | 40 Hours per Week | Monday-Friday

HumanEdge Health is seeking an experienced Travel Speech Language Pathologist (SLP) for a 13-week assignment at a Skilled Nursing Facility in Punta Gorda, Florida. This role offers the opportunity to work with a multidisciplinary team providing speech, language, and swallowing therapy to adult and geriatric patients.

Position Details

Location: Punta Gorda, FL
Setting: Skilled Nursing Facility (SNF)
Contract Length: 13 weeks
Schedule: 40 hours per week

Responsibilities

Evaluate, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders
Develop individualized treatment plans based on patient needs and clinical assessments
Provide therapy services to adult and geriatric populations within the SNF setting
Document evaluations, progress notes, and discharge summaries in accordance with facility standards
Collaborate with physicians, nursing staff, rehabilitation therapists, and family members to ensure comprehensive care
Participate in care planning meetings and interdisciplinary communication

Requirements

Active Speech Language Pathologist license (Florida or eligible)
Certificate of Clinical Competence (CCC-SLP) required
Minimum 1 year of experience as a Speech Language Pathologist
Skilled Nursing Facility (SNF) experience required
Strong communication and documentation skills
