Location: West Bridgewater, MA 
Start Date: ASAP 
Duration: 13 weeks 
Schedule: 40 hrs/week; 5x8s, 8:00 AM – 5:00 PM

Position Overview: 
HumanEdge Health is seeking an experienced Physical Therapist for a short-term travel assignment in a skilled nursing facility in West Bridgewater, MA. This role focuses on providing high-quality rehabilitative care to residents, supporting mobility, function, and overall quality of life.

Responsibilities:

<ul>
	<li>
	Evaluate, plan, and implement individualized treatment programs.
	
	</li><li>
	Collaborate with the care team to ensure comprehensive patient care.
	
	</li><li>
	Document patient progress and adjust treatment plans as needed.
	
	</li><li>
	Provide education to patients and families regarding therapy goals and exercises.
	
</li></ul>

Requirements:

<ul>
	<li>
	Current MA PT license
	
	</li><li>
	Minimum 1 year of PT experience (SNF experience preferred)
	
	</li><li>
	Current BLS certification (American Heart Association)
	
	</li><li>
	Strong clinical skills and ability to work independently
