Registered Nurse - Hospice

Location: Basking Ridge, New Jersey
Employment Type: Travel Nursing (Clinical - Nursing - Travel)
Contract Length: 21 weeks

Job Description

• Perform patient assessments to determine appropriateness for hospice care, learning needs, and progress toward goals.
• Develop, implement, and evaluate individualized plans of care for patients and families based on evidence-based knowledge.
• Maintain visit productivity standards and demonstrate knowledge of hospice regulations and insurance reimbursement guidelines.
• Complete admission, discharge, and re-visit assessments and documentation within required timeframes.
• Use age-appropriate teaching methods for patients and families; evaluate and revise teaching plans as needed.
• Assume case management responsibilities including prioritizing care needs, scheduling, and interdisciplinary communication.
• Attend and participate in interdisciplinary team (IDT) meetings every two weeks.
• Maintain a safe environment by following infection control measures and reporting infections.
• Demonstrate knowledge of pharmacology and safe medication management, including routine medication reconciliation and maintaining current medication orders.
• Participate in performance improvement programs, utilization review meetings, and provide orientation/mentorship to new employees.
• Cross-train to learn new skills and assist with agency growth through additional assignments as requested.

Schedule

• Shift: AM (11:00 AM – 7:00 PM)
• Hours per day: 7
• Hours per week: 37.5
• Every other weekend (EOW) required

Required Qualifications

• Valid RN license in New Jersey
• Minimum 2 years of hospice experience, including experience within the last year
• Experience in a home health setting
• Basic Life Support (BLS) certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing
• Valid New Jersey driver’s license

Preferred Qualifications

• Previous ICU experience

Education

• Associate's Degree in Nursing (ASN) or Diploma with RN License in New Jersey

Additional Information

• General Inpatient (GIP) experience is a plus
• Prior travel experience is not explicitly required for this position
