Case Manager - New Jersey
Job Overview
- Location: Morristown, NJ
- Employment Type: Travel, Clinical Nursing
- Contract Length: 13 weeks
- Shift: Day (8:00 AM - 4:00 PM)
- Hours: 7 hours/day, 37.5 hours/week
- Rotating weekends every 4th or 5th weekend
- Holiday coverage: 1 summer and 1 winter holiday
Job Responsibilities
- Coordinate patient care throughout acute care hospitalization
- Manage all aspects of discharge planning and transition process
- Perform utilization management and quality screenings
- In-patient case management required
- Experience with sub-acute patients required
Required Qualifications
- Valid RN license in New Jersey
- Bachelor of Science in Nursing (BSN)
- Minimum 1 year of case management experience, including within the last year
- Experience in an inpatient setting
- Acute hospital case management experience
- Acute care experience
