Job Description

• Position: Registered Nurse (RN) specializing in Medical/Surgical

Required Credentials & Certifications

Licenses & Certifications

• BLS (Basic Life Support): Must be American Heart Association (AHA) certified
• Nursys Verification: Within 30 days prior to start; disciplinary actions require FACIS Questionnaire
• Current Nursing License: Primary source verification within 30 days prior to start
• NIH Certification: Group A updated yearly; Groups B-F updated every 2 years
• MOAB Moderate Risk De-Escalation Training
• Ascension Core Competencies: HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start, updated yearly, minimum 80% score
• Ascension EKG Exam: Must pass with at least 80%, two attempts allowed
• Ascension Cerner Test-Out: Required for electronic health record competency

Personal Health & Immunizations

• MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
• Hepatitis B: Documentation of vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted
• Fit Test: Required annually; preferred prior to start
• 2 Step PPD or IGRA (Quantiferon/Tspot): Required within 90 days prior to start; TB questionnaire updated annually
• Positive PPD with Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended
• Health Statement: Statement of good health or physical exam prior to first assignment, updated annually

Background & Verification

• Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually or after >90 days break in service
• EPLS (Excluded Parties List System) Check: Within 90 days of start; rerun if break in service >90 days
• Education Verification: Must be completed within 90 days of start; diplomas not accepted, official transcripts required if verification fails
• Employment Verification: Third-party verification for past 7 years; updated annually or after >90 days break in service
• Professional References: At least two professional references required prior to first assignment

Other Requirements

• Ascension Skills Checklist: Clinical skills checklist completed within one year of start; updated annually
• Ascension 12 Panel Drug Screen: Completed within 30 days prior to start; includes testing for 12 specified substances with MRO review if needed
• Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations
• Driver’s License/State ID: Must be current and match address on file; for compact licenses, address must match home state
• Ascension Onboarding Requirements: Includes multiple education attestations and onboarding modules to be completed prior to or at start
