Required Credentials and Documentation

Submission Requirements (Must be provided before submission)

  • Current Nursing License and Primary Source Verification (within 30 days prior to start)
  • Nursys verification (within 30 days prior to start)
  • Driver's License or State ID (must not be expired, must match address on file)
  • Resume or Profile (Ascension Resume/Profile)
  • At least two professional references (completed prior to first assignment)

Onboarding Requirements (Must be completed before or at start)

  • Ascension Cerner Test-Out (Cerner test results and Training Checklist with signed attestation)
  • Ascension Badge Photo (forward-facing headshot with white or blue background, no alterations)
  • Health Statement (statement of good health or completed physical exam prior to first assignment, updated annually)
  • Fit Test (required for all travelers; preferred prior to start, can be completed onsite)
  • MMR (proof of two vaccines or positive IGG titer; no declinations accepted)
  • Hepatitis B (vaccination series completion or signed declination)
  • Varicella (proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted)
  • Tdap (valid within 10 years; no declinations accepted)
  • 2 Step PPD or IGRA (TB testing with required timing and documentation)
  • Influenza Vaccine (optional; flu season Oct 1 - Mar 31; declinations accepted with mask requirement)
  • Ascension 12 Panel Drug Screen (within 30 days prior to start, updated annually)
  • Ascension Criminal Background Check (including SSN trace, employment verification, OIG search, national sex offender check)
  • Ascension EPLS (Exclusionary Party List System) check (within 90 days of start)
  • Ascension Education Verification (within 90 days of start)
  • Ascension Employment Verification (past 7 years, updated annually)
  • Ascension Confirmation of Assignment (provided upon initial assignment and updates)
  • Ascension Skills Checklist (clinical skills, completed within one year of start, updated annually)
  • Ascension Specialty Exam (minimum 80% passing score, updated every 3 years)
  • Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80% score, updated yearly)
  • Ascension EKG Exam (minimum 80% passing score, updated as required)
  • Ascension Color Vision Test (completed prior to start, updated every 3 years)
  • Ascension Onboarding Requirements Education Attestation (completion of pre-hire 6-hour module)
  • Ascension Nursing Contract Worker Onboarding (completed onsite)
  • Ascension Nova StatStrip (pre-hire modules for non-allied positions)
  • BLS Certification (AHA required)
  • ACLS Certification (AHA required)
  • CPI/MOAB High Risk Online Portion (completed prior to start)
  • CPI/MOAB High Risk On-Site Portion (optional, completed at orientation)

Additional Notes

  • Flu vaccine exemptions/declinations require mask-wearing during flu season.
  • TB positive results require chest X-ray within one year and annual TB questionnaire.
  • Drug screen must cover all 12 specified substances with MRO review if applicable.
  • Criminal background and employment verifications must be updated annually or after breaks in service longer than 90 days.
  • Specialty and core competency exams allow only two attempts.
  • Fit testing must be current and specific mask types are required in some states.
  • All documentation must be accurate, current, and compliant with Ascension Health policies.
