Job Opportunity: CT Technologist (Travel)
Location: Hoffman Estates, Illinois
Contract Length: 13 weeks (approximately 3 months)

Facility Information
- Worksite: 1st Floor, CT Department
- Location: Hoffman Estates, IL
Required Certifications & Licenses
- BLS (Basic Life Support): Must be AHA certified.
- Current License: Must have current license with primary source verification within 30 days prior to assignment start.
- Driver's License/State ID: Must be valid, not expired, and match address requirements.
Mandatory Onboarding & Compliance Requirements
- Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year and updated yearly (minimum 80% score).
- Ascension Biometric Consent
- Ascension Non-Nursing Contingent Worker Pre-Hire Modules/Attestation (Illinois)
- Ascension Medical Imaging Contract Worker Onboarding & Education Attestation
- Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
Health & Immunization Requirements
- MMR: Documented proof of two MMR vaccines or positive IGG titer; no declinations accepted.
- Fit Test: Required for all travelers; preferred prior to start, can be completed onsite.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
- Positive PPD/Negative Chest X-Ray & Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest x-ray within a year, and complete TB questionnaire annually.
- 2 Step PPD OR IGRA: Required within 1 year and 30 days of start.
- Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted.
- Tdap: Valid within 10 years; no declinations accepted.
- Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
- Ascension 12 Panel Drug Screen: Completed within 30 days prior to start and updated annually; includes testing for 12 specified substances.
Professional & Employment Verification
- Ascension Profile (Resume)
- Two Professional References: Required prior to first assignment.
- Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender; completed within 30 days of first assignment and updated annually.
- EPLS (Excluded Parties List System) Check: Within 90 days of start, rerun if break in service > 90 days.
- Skills Checklist: Clinical skills checklist completed within one year of start and updated annually.
- Education Verification: Verification of relevant education within 90 days of start; diplomas not accepted, official transcripts required if verification fails.
- Confirmation of Assignment: Provided upon initial assignment and updates with extensions.
- Employment Verification: Third-party verification for past 7 years; updated annually or if break in service > 90 days.
Optional / Additional Requirements
- Positive PPD- Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended.
- Hiring Manager Evaluation: Post-assignment evaluation (optional).
- Name Change Documentation: Legal documentation if applicable (optional).
This position requires strict adherence to all credentialing, health, and compliance requirements prior to and during the assignment.
