Job Description

  • Work on a Behavioral Health Unit with med/surg level medical needs.
  • Unit details: 12-bed unit, 1:6 nurse-to-patient ratio.
  • Staffing: 2 nurses and 2 CNA assistive staff per shift.
  • Electronic Health Record (EHR) system: Cerner.
  • Dress code: Ceil blue scrubs.

Required Credentials and Compliance

Certifications & Licenses

  • BLS (AHA required)
  • Nursys verification within 30 days prior to start.
  • Current nursing license with primary source verification within 30 days prior to start.
  • CPI/MOAB High Risk Online Portion (Nonviolent Crisis Intervention or MOAB De-Escalation Course) completed prior to start.
  • CPI/MOAB High Risk On-Site Portion completed at orientation (optional).
  • Ascension Cerner Test-Out (testing checklist).
  • Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics) completed within a year of start, updated yearly.
  • Ascension EKG Exam with passing score (80%) required.
  • Ascension Skills Checklist (clinical skills) completed within one year of start, updated annually.

Personal Health History & Immunizations

  • MMR: Proof of two vaccines or positive IGG titer; no declinations accepted.
  • Hepatitis B: Vaccination series completion or signed declination.
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
  • Positive PPD or IGRA: Must provide positive IGRA or PPD results, negative chest X-ray within a year, and annual TB questionnaire.
  • 2 Step PPD or IGRA: Required within 30 days prior to start.
  • Varicella: Proof of two vaccines, immunity, or history; no declinations accepted.
  • Tdap: Valid within 10 years; no declinations accepted.
  • Fit Test: Required annually; preferred prior to start.
  • Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
  • Ascension 12 Panel Drug Screen: Completed within 30 days prior to start, updated annually.

Professional Documentation

  • Driver's License/State ID: Must be current and valid.
  • Ascension Profile: Resume or professional profile.
  • Ascension 2 Professional References: At least two professional references required.
  • Ascension Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually.
  • Ascension EPLS: Verification run within 90 days of start.
  • Ascension Education Verification: Verification of education relevant to clinical work within 90 days of start.
  • Ascension Confirmation of Assignment: Documentation of assignment dates.
  • Ascension Employment Verification: Third-party employment verification for past seven years.
  • Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
  • Ascension Onboarding Requirements Education Attestation: Completion of required onboarding education.
  • Ascension Nursing Contract Worker Onboarding: Completed onsite.

Additional Notes

  • Nursys and license verifications require completion of FACIS Questionnaire if any disciplinary actions exist.
  • Some checklists and attestations must be uploaded by the agency or unit leader.
  • TB questionnaire must be updated annually.
  • Flu vaccine exemptions require mask wearing at all times.
  • CPI/MOAB training includes an online portion (required) and an on-site portion (optional).
  • Drug screen must cover all 12 specified substances with MRO review if applicable.
  • Background checks and verifications must be current and updated as required.

This position is ideal for RNs with behavioral health experience who are comfortable working in a med/surg level behavioral health unit and meeting comprehensive credentialing and compliance requirements.
