Certifications & Licenses

• BLS (Basic Life Support): Must be American Heart Association (AHA) certified.
• Current License: Must have current state license with primary source verification within 30 days prior to assignment start.

Professional Requirements

• Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within one year of start and updated yearly with minimum 80% score.
• Ascension Biometric Consent: Required.
• Ascension Non-Nursing Contingent Worker Pre-Hire Modules/Attestation (Illinois): Required.
• Medical Imaging Contract Worker Onboarding Education Attestation: 6 hours pre-hire education with signed attestation required.
• Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
• Driver’s License/State ID: Must be valid, not expired, and match address requirements.

Health & Immunizations

• MMR: Documented proof of two MMR vaccines or positive IGG titer; no declinations accepted.
• Fit Test: Required for all travelers; preferred prior to start, can be completed onsite. Annual update required.
• Hepatitis B: Documentation of vaccination series completion or signed declination.

• Ascension 12 Panel Drug Screen: Completed within 30 days prior to start; includes testing for 12 specified substances with MRO review if applicable.

Background & Verification

• Ascension Profile (Resume): Required for submission.
• Professional References: At least two qualifying references prior to first assignment.
• Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender; completed within 30 days of first assignment and updated annually or after break in service >90 days.
• Exclusionary Lists (EPLS): Run within 90 days of start; rerun if break in service >90 days.
• Education Verification: Verification of relevant education within 90 days of start; rerun if break in service >90 days.
• Employment Verification: Third-party verification for past 7 years; updated annually or after break in service >90 days.
• Confirmation of Assignment: Must be provided upon initial assignment and updated with extensions.

Skills & Evaluations

• Ascension Skills Checklist: Clinical skills checklist completed within one year of start and updated annually.
