Job Requirements

• Certifications & Licenses:

  • BLS (AHA required)
  • ARRT CT certification
  • State of Wisconsin license
• Experience:

  • Experience with GE CT scanners
  • Experience with EPIC electronic health record system
• Shift:

  • Thursday: 10:30 AM – 11:00 PM
  • Friday and Saturday: 7:00 PM – 7:30 AM
• Call: None
• Holidays: Rotating with regular staff if it falls on your weekend
• Additional Duties: May need to float onsite within skill set or to sites within 40 miles if needed

Required Credentials and Onboarding Documentation

• Signed Assignment Agreement (required before onboarding)
• Current State License and primary source verification (within 30 days prior to start)
• BLS certification (AHA)
• Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; completed within a year, minimum 80% score)
• Medical Imaging Contract Worker Onboarding Education Attestation (6 hours pre-hire training)
• Health and Immunization Requirements:

  • MMR (two vaccines or positive titer; no declinations accepted)
  • Fit Test (required annually; preferred before start)
  • Hepatitis B vaccination series completion or signed declination
  • Influenza vaccine during flu season (exemptions accepted with mask requirement)
  • Varicella (two vaccines, immunity evidence, or history; no declinations accepted)
  • Tdap (valid within 10 years; no declinations accepted)
  • TB testing: 2-step PPD or IGRA, chest X-ray if positive, annual TB questionnaire
• Statement of good health or completed physical exam prior to first assignment, updated annually
• Driver’s License/State ID (must be current and match address requirements)
• Ascension Skills Checklist (clinical, completed within one year, updated annually)
• 12 Panel Drug Screen (within 30 days prior to start, updated annually)
• Education Verification (within 90 days of start)
• Employment Verification (past 7 years, updated annually)
• Criminal Background Check (including SSN trace, employment verification, OIG search, national sex offender registry; updated annually)
• EPLS check (within 90 days of start)
• Ascension Profile (resume or profile)
• Two professional references (prior to first assignment)
• Confirmation of Assignment documentation (upon initial assignment and extensions)
