Job Opportunity: RN ICU (LTA)
Location: Milwaukee, Wisconsin

Job Description
- Work every other week (EOW) and every other holiday (EOH) schedule.
- Preferred experience in CVICU and Neuro ICU, but not required.
- Will float to Neuro ICU and Burn ICU.
- Patient assignments will be within skill set; may include Med/Surg/IMC patients if needed.
- Must be able to float onsite within skill set or to other sites within 40 miles if needed.

- MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted).
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
- Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted).
- Tdap: Valid within 10 years (no declinations accepted).
- Fit Test: Required, preferred prior to start, updated annually.
- Positive PPD/Negative Chest X-Ray and Annual TB Questionnaire: Preferred but optional.
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Required within specified timeframes prior to start.
- Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
- 12 Panel Drug Screen: Required within 30 days prior to start, updated annually.
Onboarding & Testing Requirements
- Ascension Core Competencies: HIPAA, Joint Commission, OSHA topics; completed within a year of start, updated yearly.
- Ascension Skills Checklist: Clinical skills checklist completed within one year of start, updated annually.
- Ascension EKG Exam: Must pass with at least 80%, two attempts allowed.
- Ascension Epic Test-Out: Required training and test results.
- Ascension Color Vision Test: Completed prior to start and updated every 3 years.
- Ascension 2 Professional References: At least two professional references required.
- Criminal Background Check, EPLS, Employment Verification, Education Verification: Must be current and completed prior to start.
- Various Ascension-specific onboarding documents and attestations required.
Additional Notes
- Must be flexible to float between units and sites within 40 miles.
- Patient assignments may vary and include ICU, Med/Surg, IMC, Neuro, Burn units.
- Compliance with all health, licensing, and credentialing requirements is mandatory prior to assignment start.
This position is suitable for experienced ICU nurses comfortable with floating and working in a dynamic hospital environment.
