- Ascension EKG Exam with a passing score of at least 80%, updated every 3 years
- NIH Certification updated as required (Group A yearly, Group B-F every 2 years)
- MOAB Moderate Risk De-Escalation Training
- Ascension Cerner Test-Out
- Ascension Skills Checklist completed within one year of start and updated annually
- Ascension 12 Panel Drug Screen within 30 days prior to start, updated annually
- Ascension Employment, Education, and Criminal Background verifications completed prior to start and updated as required
- Health Statement or physical exam prior to first assignment, updated annually
- Driver’s License/State ID valid and matching address requirements
- Various immunizations and health requirements:
- MMR: Proof of two vaccines or positive titer, no declinations accepted
- Hepatitis B: Vaccination series completion or signed declination
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31), exemptions accepted with mask requirement
- Varicella: Proof of immunity or two vaccines, no declinations accepted
- Tdap: Valid within 10 years, no declinations accepted
- TB Screening: Positive IGRA or PPD results, negative chest X-ray within a year, and annual TB questionnaire
- Fit Test for respiratory protection, updated annually
Additional Requirements
- Resume/Profile submission
- At least two professional references prior to first assignment
- Social Security Trace, OIG Search, National Sex Offender check, and other background screenings within 30 days of first assignment
- Confirmation of assignment documentation required upon initial assignment and updates with extensions
- Post-assignment evaluations may be requested
