Job Opportunity: Occupational Therapist
Location: Tulsa, Oklahoma
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Description
- Work schedule: Monday to Friday, occasional weekends
- Setting: 100% Acute Rehab in an acute hospital environment
- No floating to other rehab settings
- Must have acute care experience
Required Qualifications
- Valid Occupational Therapist license in Oklahoma
- Occupational Therapist with 2 years of experience
- Occupational Therapist experience within the last year
- Experience in an Acute Care setting
- Acute care experience
Required Documentation & Compliance
- Professional resume/profile
- Signed assignment agreement before start
- Two professional references prior to first assignment
- Valid driver’s license or state ID (must not be expired and address must comply with licensing rules)
- Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others (updated annually or if break in service > 90 days)
- Employment verification for the past seven years (updated annually or if break in service > 90 days)
- Education verification relevant to clinical work (must be completed within 90 days of start)
- Skills checklist for specialty (to be completed within one year of start and updated annually)
- Confirmation of assignment documentation (start/end dates)
- Name change documentation if applicable
Health & Safety Requirements
- Proof of immunizations or immunity for:
- MMR (two vaccines or positive titer; no declinations accepted)
- Varicella (two vaccines, serological evidence, or verified history; no declinations accepted)
- Tdap (valid within 10 years; no declinations accepted)
- Hepatitis B vaccination series completion or signed declination
- Influenza vaccine is optional but recommended during flu season; mask required if declined
- Tuberculosis screening:
- 2-step PPD or IGRA within specified timeframes
- If positive, negative chest X-ray within one year and annual TB questionnaire
- Fit test for respiratory protection (preferred before start, can be done onsite)
- Health statement or physical exam prior to first assignment, updated annually
- 12-panel drug screen within 30 days prior to start and annually thereafter (includes amphetamines, cocaine, marijuana, PCP, methamphetamine, barbiturates, benzodiazepines, buprenorphine, methadone, oxycodone, morphine, ecstasy)
Additional Notes
- Flu season mask policy applies October 1 through March 31
- No floating to other rehab settings; strictly acute rehab
- Candidate must be prepared for occasional weekend shifts
