RN Case Manager - Nashville, Tennessee
Job Summary
Coordinate, plan, and facilitate services to meet healthcare, social, and support needs of clients. Responsibilities include assessing client needs, developing and updating individualized care plans, monitoring progress, advocating for clients, and collaborating with interdisciplinary teams to ensure optimal outcomes.
Key Responsibilities
- Conduct comprehensive assessments covering medical, social, and financial factors
- Develop, implement, and update individualized care or service plans
- Coordinate services across providers, agencies, and community resources
- Monitor client progress and adjust plans as needed
- Advocate for client access to appropriate services and resources
- Maintain accurate and timely documentation in compliance with policies and regulations
- Educate clients and families on resources and care plans
- Facilitate communication among clients, families, and healthcare/service providers
- Participate in interdisciplinary team meetings and case reviews
Contract Length
- 13 weeks
Shift
- 8:00 AM to 4:30 PM
Required Qualifications
- Valid RN license in Tennessee
- Minimum of 1 year of case management experience, including experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
