Registered Nurse (RN) Cath Lab - Night Shift - Per Diem
Location: Chicago, Illinois
Employment Type: Long-Term Assignment (LTA)
Shift: 7:30 PM – 7:00 AM
Job Overview
- Must be proficient in EPIC electronic health record system.
- Orientation and badge pick-up scheduled prior to start date.
- Availability required for a specific work period: June 11, 7:00 AM to 7:30 PM (strike coverage).
Required Qualifications
- Current RN license valid in Illinois.
- Basic Life Support (BLS) certification (American Heart Association or Red Cross).
- Advanced Cardiovascular Life Support (ACLS) certification (American Heart Association or Red Cross).
- Must pass Prime Basic Pharmacology Exam with a score of 80% or higher.
- Must pass Prime Specialty Exam with a score of 80% or higher.
- Prime Skills Checklist completion required prior to assignment.
- Prime Core Competencies Exam with a minimum score of 80%.
- Drug screening (minimum 10-panel including THC) within 60 days of start date.
- Annual fit test for respirator masks completed prior to start.
- Immunization or immunity proof for:
- COVID-19 (2 vaccines plus booster or declination)
- MMR (2 vaccines or positive titer)
- Hepatitis B (3 immunizations, immune titer, or declination)
- Influenza vaccine or declination (required Nov 1 – Mar 31)
- Varicella (vaccination or titer)
- Tdap (declinations not accepted in some states)
- Tuberculosis (TB) test within one year of start date.
- Annual TB questionnaire completed.
- Physical/health statement confirming fitness to work, completed annually or within 60 days of start.
- National criminal background check within one year of start date.
- Social Security Card or Passport for identity verification.
- Valid driver’s license, state ID, or passport.
- Prime OIG exclusion check completed.
- Prime SAM/EPLS exclusion search completed.
- Prime Agency Attestation form completed after all credentials are uploaded.
- Prime Badge Photo (clear headshot, JPEG format).
- Signed current job description (within the year).
- Onboarding education attestation completed prior to clearance to start.
- Workplace Violence Prevention training (DCI/BVP) completed.
