Required Credentials and Onboarding Requirements

Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide scanned front and back images of the card.
• License/Certificate Verification: Must be dated within 30 days of start. Attach scanned online verification with machine-generated date.

Personal Health History

• Fit Test
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferably within 14 days). Must include specific substances (no THC).
• COVID-19 Vaccine: Proof of September 2024 booster, declination, or exemption (declination not accepted in CA or IL).
• MMR: Proof of 2 vaccinations with dates or IgG titer lab results.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Influenza Vaccine: Proof of vaccine, declination, or exemption for 10/1/2024–4/30/2025 (declination not accepted in CA or IL).
• Varicella: Proof of 1 vaccination or IgG titer lab results.
• Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction; not allowed in Michigan.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.

Professional Requirements

• Trinity Code of Conduct Form: Signed page 27 required.
• Trinity I9: Standard form completed by agency.
• Confidentiality & Security Agreement: Signed page 5 required.
• Integrity and Audit Services Form
• Motor Vehicle Report (MVR): Dated within 30 days of start, showing valid license with expiration date.
• Specialty Specific Competency Testing: Specialty exam completed within the last year.
• Conflict of Interest Disclosure Form: Signed page 1 required.
• Criminal Background Check: Federal, nationwide, and county checks for last 7 years plus SSN verification, dated within 30 days of start.
• Education Verification: Highest academic degree or diploma/GED verified by third party.
• Exclusion Lists (EPLS, GSA/OIG, NSO): Reports dated within 30 days, all aliases searched.
• System Access Form: Must be completed and uploaded promptly.
• Joint Commission Testing: CORE exam within the last year.
• Skills Checklist: Provided by agency for clinical contractors.

• Trinity Profile: Resume, skills checklist within a year, and 2 professional references required for submission.
