- ARRT-CT certification required
Required Credentials & Onboarding Requirements
Personal Health History
- COVID-19 Vaccine: Proof of September 2025 booster, declination or exemption with agency letterhead required. Declination not accepted for CA or IL.
- MMR: Proof of 2 vaccinations with dates or IgG titer lab report required.
- Fit Test: Required (Iowa - 1860 mask only).
- Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Hep B titers not accepted.
- Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead required. Declination not accepted for CA or IL. Flu season dates apply.
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Required. Positive TB requires chest x-ray within last year for Michigan.
- Varicella: Proof of 1 vaccination with date or IgG titer lab report required.
- Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
- Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include 10 specific substances; no THC allowed.
- Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.
Certifications
- BLS: American Heart Association certification required for all direct patient care personnel. Must provide front and back images of card with expiration date.
Licenses
- License/Certificate Verification: Must be dated within 30 days of start. Online verification with machine-generated date required. For RNs/LPNs, state verification (e.g., NURSYS) required. California requires picture of hard copy license for RN.
Professional Requirements
- Trinity Profile: Resume, skills checklist within a year, and 2 professional references required for submission.
- Confidentiality Agreement: Signed Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement required.
- Conflict of Interest Disclosure: Signed form required.
- Criminal Background Check: Federal, nationwide, county criminal check covering last 7 years plus SSN verification, dated within 30 days of start. Includes employment verifications.
- Education Verification: Third-party screening report of highest academic education required. Certifications must be provided if required by job description.
- EPLS (Excluded Parties List System): Scanned report dated within past 30 days required.
- GSA/OIG Exclusion Check: Scanned report dated within past 30 days required.
- Integrity and Audit Services Form: Signed within days prior to start.
- Joint Commission Testing: CORE exam within the last year required.
- NSO (National Sex Offender) Check: Dated within 30 days of start, including all aliases and locations. Michigan requires statewide check as well.
- System Access Form: Must be completed and uploaded one week before start.
- Skills Checklist: Provided by agency for clinical contractors; required for onboarding.
- Code of Conduct Form: Signed form required.
- I9 Form: Standard I9 completed by agency.
- Specialty Specific Competency Testing: Specialty exam completed within last year required.
