Job Description

• Work in a busy Emergency Room setting
• Must work every other weekend
• Required certifications: BLS, ACLS, PALS, NIHSS

Required Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel
• ACLS (Advanced Cardiovascular Life Support): Required for RNs in ED
• PALS (Pediatric Advanced Life Support): Required for RNs in ED; ENPC certificate also acceptable
• TNCC (Trauma Nursing Core Course): Required
• License/Certificate Verification: Must be dated within 30 days of start; state verification required (e.g., NURSYS for RNs)

Health & Safety Requirements

• Fit Test
• Trinity Respirator Evaluation: Physician signature required
• Trinity 10 Panel Drug Screen: Completed within 30 days of start, must include specific substances (no THC)
• COVID-19 Vaccine: Proof of vaccine or approved declination/exemption required (with agency letterhead)
• Influenza Vaccine: Proof of vaccine, declination, or exemption required for current flu season
• MMR: Proof of 2 vaccinations or IgG titer lab results
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted
• Varicella: Proof of vaccination or IgG titer lab results
• 2 Step PPD OR IGRA (Quantiferon/Tspot): TB screening with chest x-ray if positive, per regional requirements
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work

Professional & Compliance Requirements

• Trinity Code of Conduct Form: Signed and on file
• Trinity I9: Completed by agency
• Confidentiality & Security Agreement: Signed form required
• Criminal Background Check: Federal, nationwide, county checks for last 7 years; SSN verification; dated within 30 days of start
• Education Verification: Highest academic degree or diploma/GED verified
• EPLS (Excluded Parties List System) Check: Dated within 30 days of start
• OIG (Office of Inspector General) Check: Dated within 30 days of start
• Integrity and Audit Services Form: Signed shortly before start
• Joint Commission CORE Exam: Completed within last year
• NSO (National Sex Offender) Check: Dated within 30 days of start; includes aliases and statewide checks where applicable
• System Access Form: Completed and uploaded one week before start
• Conflict of Interest Disclosure Form: Signed and on file
• OFAC (Office of Foreign Assets Control) Sanctions Check: Completed within 30 days of start
• Job Description: Signed copy uploaded
• Skills Checklist: Provided by agency for clinical contractors
