Required Experience

Personal Health History / Immunizations

• MMR (proof of 2 vaccinations or IgG titer)
• Fit Test
• Hepatitis B vaccination series (1st shot prior to start, 2nd shot 30 days later, 3rd shot 6 months later; titers not accepted)
• Influenza Vaccine (proof or declination/exemption; declination not accepted for CA or IL)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (timing depends on state)
• Varicella (proof of 1 vaccination or IgG titer)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Trinity 10 Panel Drug Screen (completed within 30 days of start; no THC)

Professional Requirements

• 2 Professional References
• Confidentiality and Security Agreement
• Conflict of Interest Disclosure Form
• Criminal Background Check (Federal, Nationwide, County for last 7 years; includes SSN verification)
• Education Verification (highest academic degree or diploma/GED)
• EPLS (Excluded Parties List System) check within 30 days
• GSA/OIG Exclusion check within 30 days
• Integrity and Audit Services Form (signed shortly before start)
• Joint Commission CORE Exam (within last year)
• National Sex Offender Registry (NSO) check within 30 days
• System Access Form (completed and uploaded one week before start)
• Specialty Specific Competency Testing (completed within last year)

Certifications & Licenses

• BLS (American Heart Association certification; front and back of card required)
• Current License/Certificate Verification (dated within 30 days of start; state verification required)

Additional Requirements

• Trinity Code of Conduct Form (signed)
• Standard I9 form completed by agency
• Trinity Color Vision test

Summary

This is a 13-week CT Technologist contract in Ohio with a Monday-Thursday 4x10 schedule.
