Job Opportunity: CT Technologist

Location: Maywood, Illinois
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks (approximately 3 months)
Schedule: Includes every third weekend

Required Credentials & Certifications

• BLS (Basic Life Support) – American Heart Association certification required for all direct patient care personnel
• ARRT Certification
• State of Illinois License
• Fit Test (personal health requirement)
• Trinity 10 Panel Drug Screen (must be completed within 30 days of start)
• COVID-19 Vaccine (proof of booster, declination not accepted in IL)
• MMR Vaccination (proof of 2 vaccinations or IgG titer)
• Hepatitis B Vaccination (must have started series prior to start)
• Influenza Vaccine (required for the flu season)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) (TB test)
• Varicella Vaccination (proof of 1 vaccination or IgG titer)
• Physical/Health Statement (documentation from exam within last year)

Professional & Compliance Requirements

• Trinity Code of Conduct Form
• Trinity I9 Form
• Confidentiality of Patient & Proprietary Information and Information System Security Agreement
• Integrity and Audit Services Form
• Motor Vehicle Report (MVR) – must be dated within 30 days of start
• Trinity Profile (Resume, Skills Checklist within a year, 2 Professional References)
• Conflict of Interest Disclosure Form
• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification)
• Education Verification (highest level of academic education)
• EPLS (Excluded Parties List System) Check
• GSA/OIG Exclusion Check
• Joint Commission Testing (CORE Exam within a year)
• NSO (National Sex Offender) Check
• System Access Form (must be completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
