• Trinity 10 Panel Drug Screen (completed within 30 days of start; no THC)

• Resume and Skills Checklist (within the last year)
• Confidentiality and Security Agreement
• Conflict of Interest Disclosure Form
• Criminal Background Check (federal, nationwide, county for last 7 years; dated within 30 days)
• Education Verification (highest academic degree or diploma verified)
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG Exclusion Check dated within 30 days
• Integrity and Audit Services Form (signed shortly before start)
• Joint Commission CORE Exam (within last year)
• National Sex Offender Registry (NSO) check dated within 30 days
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (agency provided for clinical contractors)
• License/Certificate Verification (dated within 30 days; state verification required)
• BLS Certification (American Heart Association; front and back of card required)
