Respiratory Therapist Position - Hartford, Connecticut
Job Details
- Employment Type: Long-Term Assignment (LTA)
- Shift: 7:00 PM – 7:30 AM
- Contract Length: 13 weeks
Required Qualifications
- Valid Respiratory Therapist license in Connecticut
- Minimum of 1 year of Respiratory Therapist experience, including experience within the last year
- BLS issued by the American Heart Association
- ACLS certification
Health and Safety Requirements
- Fit Test
- COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption except for CA or IL)
- MMR (proof of 2 vaccinations or IgG titer)
- Hepatitis B vaccination series (at least first shot prior to start)
- Influenza Vaccine (optional)
- 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (within last year for CT)
- Varicella (proof of vaccination or IgG titer)
- Physical/Health Statement (exam within last year confirming fitness to work)
- Trinity 10 Panel Drug Screen (completed within 30 days of start; no THC)
Professional and Compliance Requirements
- Signed Code of Conduct form
- Completed I9 form
- Confidentiality and Information Security Agreement
- Integrity and Audit Services Form (signed shortly before start)
- Specialty Specific Competency Testing (completed within last year)
- Skills Checklist (provided by agency for clinical contractors)
- Criminal Background Check (federal, nationwide, county for last 7 years; SSN verification; dated within 30 days)
- Education Verification (highest academic degree or diploma/GED)
- EPLS (Excluded Parties List System) check within 30 days
- OIG (Office of Inspector General) exclusion check within 30 days
- Joint Commission CORE Exam (within last year)
- NSO (National Sex Offender) check within 30 days
- System Access Form (completed and uploaded one week before start)
- Conflict of Interest Disclosure Form
- OFAC (Office of Foreign Assets Control) sanctions check within 30 days
