CT Technologist Position - Indiana, Mishawaka

Job Details

• Job Title: CT Technologist
• Location: Mishawaka, Indiana
• Employment Type: Long-Term Assignment (LTA)
• Contract Length: 13 weeks
• Shift: Monday through Thursday, 8:30 AM to 7:00 PM

Required Qualifications

• Valid CT Technologist license in Indiana
• Minimum of 1 year of CT Technologist experience within the last year
• BLS certification issued by the American Heart Association
• ARRT (CT) certification

Onboarding and Compliance Requirements

• Current BLS certification (American Heart Association), including front and back card images
• State license verification dated within 30 days of start
• Fit test
• Annual TB questionnaire and 2-step PPD or IGRA test (with chest x-ray if positive TB as per regional requirements)
• Trinity 10-panel drug screen completed within 30 days of start (no THC)
• Proof of COVID-19 vaccination or approved exemption/declination (booster required by Sept 2025; declination not accepted in CA or IL)
• Proof of MMR vaccination (2 doses) or IgG titer lab results
• Hepatitis B vaccination series started prior to start date (titers not accepted)
• Proof of Varicella vaccination or IgG titer
• Physical/health statement confirming fitness to work within the last year
• Trinity color vision and latex allergy questionnaires
• Tdap vaccine or declination if no pediatric interaction

Professional and Background Checks

• Signed Code of Conduct form
• Completed I-9 verification
• Confidentiality and system security agreement
• Integrity and audit services form signed shortly before start
• Joint Commission CORE exam within the last year
• Conflict of Interest disclosure form
• Federal, nationwide, and county criminal background check covering last 7 years, including SSN verification (dated within 30 days of start)
• Education verification of highest academic degree or diploma/GED
• EPLS (Excluded Parties List System) check dated within 30 days
• OIG (Office of Inspector General) exclusion check dated within 30 days
• NSO (National Sex Offender) check dated within 30 days, including aliases and statewide checks for Michigan if applicable
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days
• System access form completed and uploaded one week before start
• Trinity orientation guide completion

Additional Notes

• Resume and skills checklist (within one year) and two professional references required for submission
• Agency will provide skills checklist for clinical contractors
• Influenza vaccine is optional but recommended (declination accepted except in CA or IL for flu season 10/1/2024–4/30/2025 and 10/1/2025–4/30/2026)
