Description

Job Opportunity: CT Technologist

Location: Ann Arbor, Michigan
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Job Details

  • Includes up to every other week (EOW) shifts
  • Up to one on-call shift per month
  • Includes any holidays that fall during the contract

Required Credentials & Onboarding Requirements

Certifications

  • BLS (Basic Life Support)
    • American Heart Association certification required for all direct patient care personnel
    • Must provide scanned copy of certification (front and back) with expiration date

Licenses

  • License/Certificate Verification
    • Must be dated within 30 days of start
    • Attach scanned online verification with machine-generated date within past 30 days
    • For RNs and LPNs, upload state verification (e.g., NURSYS)
    • California only: also provide picture of hard copy license for RN

Personal Health History Requirements

  • COVID-19 Vaccine
    • Proof of booster by September 2025, or declination/exemption with agency letterhead
    • Declination not accepted for CA or IL
  • MMR (Measles, Mumps, Rubella)
    • Proof of 2 vaccinations with dates or IgG titer lab report
  • Fit Test
    • Required (Iowa: 1860 mask only)
  • Hepatitis B
    • Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later)
    • Hep B titers NOT accepted
  • Influenza Vaccine
    • Proof of vaccine, declination, or exemption with agency letterhead
    • Declination not accepted for CA or IL
    • Flu season dates: 10/1/2024 - 4/30/2025 and 10/1/2025 - 4/30/2026
  • 2 Step PPD OR IGRA (Quantiferon/Tspot)
    • Positive TB requires chest x-ray within last year for Michigan and certain states, within 5 years for others
  • Varicella
    • Proof of 1 vaccination with date or IgG titer lab report
  • Physical/Health Statement
    • Documentation from exam within last year confirming fitness to work
  • Tdap
    • Current, non-expired vaccine required
    • Declinations accepted if no pediatric interaction
  • Trinity 10 Panel Drug Screen
    • Completed within 30 days of start (preferred within 14 days)
    • Must include: Cocaine, Amphetamines, Opiates, 6-Monoacetylmorphine, Propoxyphene, PCP, Barbiturates, Benzodiazepines, Methaqualone, Methadone
    • NO THC allowed
  • Trinity Color Vision
    • Required (no additional comments)

    • Resume, skills checklist within a year, 2 professional references required for submission
  • Confidentiality & Security Agreements
    • Signed Trinity Confidentiality of Patient & Proprietary Information and System Security Agreement
    • Signed Conflict of Interest Disclosure Form
    • Signed Code of Conduct Form
  • Background Checks & Verifications
    • Criminal background check (federal, nationwide, county) covering last 7 years, dated within 30 days of start
    • SSN verification included
    • Employment verifications covering last 7 years
    • EPLS (Excluded Parties List System) report dated within 30 days
    • OIG (Office of Inspector General) exclusion report dated within 30 days
    • NSO (National Sex Offender) check dated within 30 days, including statewide check for Michigan
  • Education Verification
    • 3rd party screening report verifying highest academic education level (degree or HS diploma/GED)
  • Integrity and Audit Services Form
    • Signed within days prior to start
  • Joint Commission Testing
    • CORE exam within the last year
  • System Access Form
    • Must be completed and uploaded one week before start
  • I9 Form
    • Standard I9 completed by agency
  • Specialty Specific Competency Testing
    • Specialty exam completed within the last year
  • Skills Checklist
    • Provided by agency for clinical contractors (not required for non-clinical)
