Job Summary
- Provide direct nursing care following established policies, procedures, and protocols.
- Implement and monitor patient care plans.
- Monitor, record, and communicate patient condition as appropriate.
- Coordinate all disciplines for well-coordinated patient care.
- Follow physician and nursing orders.
- Assess and coordinate patient discharge planning with healthcare team members.
- Requires critical thinking, decisive judgment, and ability to work with minimal supervision in a stressful environment.
- Use personal protective equipment (PPE) following Standard Precautions.
Required Credentials & Compliance
Certifications & Licenses
- BLS (Basic Life Support): Must be American Heart Association (AHA) certified.
- Nursys Verification: Within 30 days prior to start; disciplinary actions require FACIS Questionnaire.
- License/Certificate Verification: Current license and primary source verification within 30 days prior to start.
- Driver's License/State ID: Must be valid and match address requirements.
Exams & Testing
- Ascension Pharmacology Exam: Pass with minimum 80%, updated every 3 years.
- Ascension Specialty Exam: Pass with minimum 80%, updated every 3 years.
- Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; pass with minimum 80%, updated yearly.
- Ascension Cerner Test-Out: Required for electronic health record competency.
- Ascension Skills Checklist: Clinical skills checklist, updated annually.
- Ascension 12 Panel Drug Screen: Completed within 30 days prior to start, updated annually.
Health & Immunizations
- MMR: Proof of two vaccines or positive titer; no declinations accepted.
- Hepatitis B: Vaccination series completion or signed declination.
- Varicella: Proof of immunity or two vaccines; no declinations accepted.
- Tdap: Valid within 10 years; no declinations accepted.
- Influenza Vaccine: Optional but recommended; exemptions require mask-wearing during flu season.
- Fit Test: Required for all travelers; updated annually.
- 2 Step PPD or IGRA (TB Test): Required with annual TB questionnaire; chest X-ray if positive.
- Health Statement: Statement of good health or physical exam prior to assignment, updated annually.
- Ascension Color Vision: Completed prior to start and updated every 3 years.
Professional Requirements
- Ascension Profile: Resume or professional profile submission.
- Professional References: At least two qualifying references prior to first assignment.
- Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually.
- Employment Verification: Third-party verification for past 7 years; updated annually.
- Education Verification: Verification of relevant education within 90 days prior to start.
- Ascension Confirmation of Assignment: Provided upon initial assignment and updates with extensions.
- Ascension Badge Photo: Forward-facing headshot with white or blue background.
Onboarding & Training
- Ascension Onboarding Requirements Education Attestation: Completion of pre-hire education.
- Ascension Nursing Contract Worker Onboarding: Onsite completion required.
- Ascension Nova StatStrip: Pre-hire modules for non-allied positions.
