Job Opportunity: CT Technologist

Location: Columbus, Ohio

Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule

• 4 shifts per week, 10 hours each
• Monday to Thursday
• Shift options: 12:00 PM – 10:30 PM or 2:00 PM – 12:30 AM

Required Credentials and Onboarding Documentation

Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide front and back images of the card.
• License/Certificate Verification: Must be dated within 30 days of start. Online verification required.

Health & Safety

• Fit Test
• Trinity 10 Panel Drug Screen: Completed within 30 days of start, includes testing for multiple substances (NO THC).
• COVID-19 Vaccine: Proof of vaccine, declination, or exemption required (declination not accepted in CA or IL).
• MMR: Proof of 2 vaccinations or lab IgG titer.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Influenza Vaccine: Optional but recommended (proof or declination accepted).
• Varicella: Proof of vaccination or lab IgG titer.
• Tdap: Current, non-expired vaccine required; declination accepted if no pediatric interaction.
• Physical/Health Statement: Exam within the last year confirming fitness to work.
• 2 Step PPD OR IGRA (Quantiferon/Tspot)

• Trinity Code of Conduct Form
• Trinity I9 Form
• Confidentiality & System Security Agreement
• Integrity and Audit Services Form
• Conflict of Interest Disclosure Form
• Criminal Background Check: Federal, nationwide, county checks for last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification: Highest academic degree or diploma verified.
• EPLS (Excluded Parties List System) Report: Dated within 30 days.
• GSA/OIG Exclusion Check: Dated within 30 days.
• NSO (National Sex Offender) Check: Dated within 30 days.
• System Access Form: Must be completed and uploaded one week before start.
• Trinity Specific COVID Questionnaire
• Specialty Specific Competency Testing: Must be completed within the last year.
• Skills Checklist: Provided by agency for clinical contractors.

• Resume and professional references required for submission.
• All documentation must be uploaded into TrinityVMS system as part of onboarding.
