Required Credentials & Onboarding Information
Licenses & Certifications
- Current Nursing License: Must have primary source verification within 30 days prior to start.
- Nursys Verification: Required within 30 days prior to start.
- BLS (AHA): Required prior to start.
- ACLS (AHA): Required prior to start.
- CPI/MOAB High Risk Online Portion: Must be completed prior to start.
- Additional Certifications: NIH Certification (updated yearly or every 2 years depending on group).
Professional Documentation
- Resume/Profile (required for submission)
- Two professional references (required for submission)
- Criminal background check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender Registry (required for onboarding)
- Employment verification for past 7 years (required for onboarding)
- Education verification relevant to clinical work (required for onboarding)
- Confirmation of assignment documentation (required for onboarding)
- Driver’s License/State ID (must be current and match address requirements)
Testing & Checklists
- Ascension Cerner Test-Out (required for onboarding)
- Ascension Skills Checklist (clinical skills, updated annually)
- Ascension Specialty Exam (minimum 80% passing score, updated every 3 years)
- Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80% score, updated yearly)
- Ascension EKG Exam (minimum 80% passing score, updated as of Dec 2023)
- Ascension Onboarding Requirements Education Attestation (completion of pre-hire training)
- Ascension Nursing Contract Worker Onboarding (completed onsite)
- Ascension Nova StatStrip pre-hire modules (required for non-allied positions)
Health & Immunization Requirements
- MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
- Varicella: Proof of two vaccines, serological immunity, or verified history (no declinations accepted)
- Tdap: Valid within 10 years (no declinations accepted)
- Hepatitis B: Documentation of vaccination series completion or signed declination
- Influenza Vaccine: Optional; if declined during flu season (Oct 1 - Mar 31), mask required at all times
- Positive PPD: Negative chest X-ray within 1 year and annual TB questionnaire (if applicable)
- 2 Step PPD or IGRA: Required within specified timeframes prior to start
- Fit Test: Required for all travelers; preferred prior to start, can be completed onsite (PAPR not done onsite)
- Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually
- Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually
Additional Requirements
- Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations
- Ascension Criminal Background Check: Updated annually or if break in service > 90 days
- Ascension EPLS (Excluded Parties List System) check: Within 90 days of start, rerun if break in service > 90 days
- Ascension Employment Verification: Updated annually or if break in service > 90 days
- Ascension Name Change Document: Optional, if applicable
- Hiring Manager Evaluation: Optional post-assignment evaluation
- Ascension Unit Specific Checklist: Optional for onboarding, uploaded by unit leader

This is a 13-week ICU RN contract position at Ascension Via Christi St. Francis in Wichita, Kansas.
