Job Description
- Position in Cardiothoracic Intensive Care Unit (ICU)
- Requires specialized critical care nursing skills
Required Credentials & Onboarding Requirements
Submission Requirements (Before Assignment)
- Current Nursing License with primary source verification (within 30 days prior to start)
- Nursys license verification (within 30 days prior to start)
- Professional resume/profile
- At least two professional references
- Valid Driver’s License or State ID (must not be expired and must meet address requirements)
Onboarding Requirements (Must be completed prior to or at start)
- Ascension Cerner Test-Out (clinical system competency)
- Ascension Badge Photo (headshot with white or blue background, no alterations)
- Health and Immunization Documentation:
- MMR: Proof of two vaccines or positive titer (no declinations accepted)
- Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
- Hepatitis B: Vaccination series completion or signed declination
- Tdap: Valid within 10 years (no declinations accepted)
- Influenza Vaccine (optional but recommended during flu season; declinations require mask wearing)
- 2 Step PPD or IGRA testing for TB, with annual TB questionnaire
- Positive PPD requires negative chest X-ray within 1 year and annual TB questionnaire
- Fit Test for respiratory protection (preferred before start, can be done onsite; PAPR not done onsite)
- Statement of good health or completed physical exam (updated annually)
- Ascension 12 Panel Drug Screen (within 30 days prior to start, updated annually)
- Criminal Background Check (including SSN trace, employment verification, OIG, sex offender registry)
- Employment Verification for past 7 years
- Education Verification relevant to clinical work (within 90 days prior to start)
- Ascension Skills Checklist (clinical skills, updated annually)
- Ascension Specialty Exam (minimum 80% passing score, updated every 3 years)
- Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80% score, updated yearly)
- Ascension EKG Exam (minimum 80% passing score, updated as required)
- Ascension Color Vision Test (updated every 3 years)
- BLS and ACLS certifications (must be American Heart Association certified)
- NIH Certification (Group A yearly, Groups B-F every 2 years)
- CPI/MOAB High Risk Online Portion (nonviolent crisis intervention and de-escalation training prior to start)
- Ascension Onboarding Education Attestation (completion of pre-hire training modules)
- Ascension Nursing Contract Worker Onboarding (completed onsite)
- Ascension Nova StatStrip pre-hire modules (for non-allied positions)

- Ascension Unit Specific Checklist (optional, uploaded by unit leader)
- CPI/MOAB High Risk On-Site Portion (completed at orientation)
