Job Description
- Start dates requested: 10/20 or 11/3
- Shift options:
- 4x10 or 4x12 hour shifts, Day or Evening rotations
- Day shifts (4x10):
- Monday, Tuesday, Thursday, Friday: 7:00 AM – 5:30 PM
- Wednesday: 9:00 AM – 7:30 PM
- Evening shifts (4x10): 1:00 PM – 11:00 PM
- Day shifts (4x12):
- Monday, Tuesday, Thursday, Friday: 7:00 AM – 7:30 PM
- Wednesday: 9:00 AM – 9:30 PM
- Evening shifts (4x12): 11:00 AM – 11:00 PM
- Weekend shifts included; when scheduled on weekends, shifts will be 12 hours, with 8 or 10 hour shifts during the same week to meet contracted hours
- Includes 2 weeks of night shifts per contract
- At least 2 short calls per 4-week schedule and weekend coverage required
- Must have cardiac scrub/circulate skillset
- Holiday availability required
Required Credentials & Compliance
Professional Credentials
- MGB Badge Photo: Color photo taken within past 6 months (no selfies, plain background, symmetrical headshot)
- License/Certificate Verification: Primary source verification of professional nursing license (NURSYS and BON required). MA Conditional Licenses not accepted.
- Resume: Past 7 years of work experience with city & state details
- References: 2 recent references, minimum 1 supervisor or above, in relevant skillset
- Skills Checklist: Required for submission
- Confirmation of Assignment/POI Form: Required upon acceptance and for extensions
- Criminal Background Check: Within 45 days prior to start, including international searches if applicable
- Massachusetts CORI Check: Within 30 days prior to start, updated if >6 month break
- Education Verification: Verified through background check company (highest level achieved)
- SSN Trace: Must be run with criminal check, covering last 7 years
- National Sex Offender Check: Within 30 days of start, updated annually
- Exclusion Checks: OIG, GSA, MassHealth & NH Medicaid exclusion lists, updated annually
Personal Health History & Testing
- Fit Test: N95 fit test within 365 days of start, updated annually (no religious exemptions)
- MMR: Documentation of two measles, two mumps, and one rubella vaccine OR proof of immunity via blood test
- TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start, updated if >6 month break
- Health Statement: Physician statement within 365 days of start, updated if >6 month break
- Respirator Medical Clearance Attestation: Required if fit test is required
- Influenza Vaccine: Required during flu season (Sept 15 – April 30), no exemptions
- Optional but Preferred: Hepatitis B vaccination or immunity proof, Tdap vaccine within 10 years
- Positive PPD: If positive TB test, negative chest x-ray documentation required
- TB Symptom Survey: If positive PPD, must complete screening form prior to start and annually
Additional Notes
- Must be available for holiday shifts
- Schedule includes a mix of day, evening, night, weekend, and short call shifts
- Cardiac scrub/circulate experience is mandatory
