Work Schedule

• 6:00 AM to 6:00 PM shifts
• Every other weekend
• Holiday rotation

Required Certifications and Licenses

• BLS (American Heart Association certification required for all direct patient care personnel)
• ARRT-CT certification
• Current license/certification verification dated within 30 days of start

Health and Safety

• Fit Test
• Trinity 10 Panel Drug Screen (9 Panel, no THC, completed within 30 days of start)
• COVID-19 Vaccine (Proof of September 2024 booster, declination or exemption accepted except for CA or IL)
• MMR (Proof of 2 vaccinations or IgG titer)
• Hepatitis B vaccination series (1st shot prior to start, 2nd shot 30 days later, 3rd shot 6 months later; titers not accepted)
• Influenza Vaccine (optional, proof or declination accepted except for CA or IL; valid 10/1/2024-4/30/2025)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive (timing depends on state)
• Varicella (Proof of 1 vaccination or IgG titer)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Tdap vaccine (current, non-expired; declinations accepted if no pediatric interaction)

Professional and Compliance Forms

• Trinity Code of Conduct Form (signed)
• Trinity I9 form (completed by agency)
• Confidentiality of Patient & Proprietary Information and System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non Employee Upload Form (pages 1-3 and 28 completed and uploaded)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification; dated within 30 days of start)
• Education Verification (highest academic degree or diploma/GED verified)
• EPLS (Excluded Parties List System) report dated within 30 days
• GSA/OIG Exclusion report dated within 30 days
• Joint Commission CORE Exam (within last year)
• NSO (National Sex Offender) check dated within 30 days, including aliases and locations
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
• Child and Adult Dependent Abuse Training (required for Iowa)
• Iowa Sing Report (Iowa-specific criminal history, sex offender, dependent adult & child abuse registries check within 30 days prior to start)

Summary

This is a 13-week CT Technologist contract position at MercyOne Clinton Medical Center in Iowa with Trinity Health. The role requires ARRT-CT certification, BLS certification, and a variety of health, safety, and professional compliance credentials to be completed and verified prior to or at onboarding. The schedule includes 12-hour day shifts with every other weekend and holiday rotations.

Candidates must be prepared to provide extensive documentation including immunizations, drug screening, background checks, and compliance forms as outlined above.
