Licenses & Certifications

• License/Certificate Verification: Primary source verification required within 30 days of start date; MA Conditional Licenses not accepted.
• ARRT Certification required.
• MGB Education Verification: Verification of highest education level achieved.

Health & Immunization Requirements

• Fit Test (N95 respirator): Must be completed within 365 days prior to start and updated annually. No religious exemptions.
• MMR Vaccine: Documentation of two measles, two mumps, and one rubella vaccine OR two MMR vaccines OR proof of immunity via blood test.
• Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions allowed.
• TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date; updated if break >6 months.
• Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start date; updated if break >6 months.
• Tdap Vaccine: Preferred within last 10 years.
• Hepatitis B: Strongly preferred; record of 3 vaccines or proof of immunity or declination.
• Positive PPD with Negative Chest X-Ray: Required if applicable.
• Respirator Medical Clearance Attestation: Required if fit test is required.

Background & Professional Checks

• Criminal Background Check: Within 45 days prior to start; updated if break >6 months. Includes international searches if applicable.
• Massachusetts CORI Check: Within 30 days prior to start; updated if break >6 months.
• National Sex Offender Check: Within 30 days prior to start; updated annually.
• Exclusion Checks: Within 30 days prior to start and annually; includes OIG, GSA, MassHealth & NH Medicaid lists.
• SSN Trace: Must be run with criminal check covering last 7 years.
• Education Verification: Verified through background check company.

Documentation & Submission Requirements

• Resume: Include past 7 years of work experience with city & state. Required for submission.
• References: Two recent references required, at least one supervisor or above in skillset.
• Skills Checklist: Required for submission.
• Candidate Submission Coversheet: Required for submission.
• Confirmation of Assignment/POI Form: Required for all initial assignments and extensions upon acceptance.
