Location: Elgin, Illinois

Shift: Dayshift, weekdays with on-call required
Contract Length: 13 weeks

Job Description

• Work in Cath Lab, Interventional Radiology (IR), and Electrophysiology (EP) Lab
• Must be cross-trained in all three areas
• On-call availability required

Required Credentials & Onboarding Requirements

Professional Credentials

• Valid Driver's License, State ID, or Passport (required for onboarding)
• Prime OIG clearance
• Prime SAM/EPLS verification
• Resume submission required
• Two professional references required
• Social Security Card
• Criminal Background Check (National, within one year of start)
• Badge photo (clear headshot, JPEG format)
• Form I-9 (Employment eligibility verification)
• Signed Job Description
• Certificate of Authenticity (COA)
• Education Verification
• Essentials Onboarding Packet completion

Certifications

• DCI/BVP (Bloodborne Pathogens)
• BLS (American Heart Association or Red Cross)
• ACLS (American Heart Association or Red Cross)

Personal Health History & Testing

• Drug Screening: Minimum 10-panel test including THC, within 60 days of start
• COVID-19 Vaccine: 2 doses plus booster or completed declination
• MMR: Proof of 2 vaccines or positive titer
• Influenza Vaccine: Required October 1 - May 31 or declination
• Hepatitis B: Vaccination required (declinations not accepted at some locations)
• Tdap: Vaccination required (declinations not accepted at some locations)
• Varicella: Vaccination or titer showing immunity
• TB Test and Annual TB Questionnaire
• Fit Test: Annual respirator fit test required
• Physical/Health Statement: Annual, must state clear to work

Exams & Checklists

• Prime Core Competencies Exam (minimum 80%)
• Prime Skills Checklist (submission required)
• Prime Basic Pharmacology Exam (minimum 80%)
• Prime Specialty Exam (minimum 80%)
• Prime Agency Attestation (completed after all credentials uploaded)

Additional Notes

• Cross-training in Cath Lab, IR, and EP Lab is mandatory
• On-call duties are part of the schedule
• Dayshift, weekdays only
• Contract length is approximately 13 weeks

This position is suitable for experienced RN Cath Lab nurses who are flexible with on-call duties and have the required certifications and clearances.
