Job Opportunity: CT Technologist
Location: Ann Arbor, Michigan
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Work Schedule
- As often as every other week (EOW)
- Approximately 1 call shift per month
Required Certifications & Credentials
- BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide scanned copy of both front and back of card with expiration date.
- License/Certificate Verification: Must be dated within 30 days of start. Provide scanned online verification with machine-generated date. For RNs and LPNs, upload state verification (e.g., NURSYS). California requires a picture of the hard copy license for RNs.
Health & Immunization Requirements (Must be completed prior to onboarding)
- COVID-19 Vaccine: Proof of vaccine including September 2025 booster, or declination/exemption with agency letterhead (declination not accepted for CA or IL). Valid through October 2026.
- MMR: Proof of 2 vaccinations with dates or primary source IgG titer lab report.
- Fit Test: Required (Iowa - 1860 mask only).
- Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Hep B titers not accepted.
- Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead. Declination not accepted for CA or IL. Flu season dates apply (e.g., 10/1/2024 - 4/30/2025).
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Positive TB requires chest x-ray within last year for Michigan and certain states; otherwise within 5 years.
- Varicella: Proof of 1 vaccination with date or primary source IgG titer lab report.
- Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction; agency form acceptable.
- Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
Drug Screening
- Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferably within 14 days). Must include cocaine, amphetamines, opiates, 6-monoacetylmorphine, propoxyphene, PCP, barbiturates, benzodiazepines, methaqualone, and methadone. No THC allowed. Must have MRO signature.
Professional Requirements (Must be completed prior to onboarding)
- Trinity Profile: Resume, skills checklist within the past year, and 2 professional references required for submission.
- Confidentiality & Security Agreements: Signed confidentiality and proprietary information agreements required.
- Conflict of Interest Disclosure Form: Must be signed and on file.
- Criminal Background Check: Federal, nationwide, and county checks covering last 7 years, including SSN verification, dated within 30 days of start. Employment verifications for last 7 years required.
- Education Verification: Third-party screening report verifying highest academic education (degree or high school diploma/GED). Certifications required if specified by job description.
- Exclusion and Sanctions Checks: EPLS, OIG, NSO reports dated within 30 days of start, including all aliases. Michigan requires statewide checks as well.
- Integrity and Audit Services Form: Signed within days prior to start.
- Joint Commission Testing: CORE exam within the past year.
- System Access Form: Must be completed and uploaded one week before start.
- Code of Conduct Form: Signed form required.
- I9 Verification: Standard I9 completed by agency.
- Specialty Specific Competency Testing: Specialty exam completed within the past year.
- Skills Checklist: Provided by agency for clinical contractors.
