Required Certifications & Licenses

• BLS (Basic Life Support) – American Heart Association certification required for all direct patient care personnel
• ARRT (R)(CT) – Required for submission
• License/Certificate Verification – Must be dated within 30 days of start, with online verification attached

Health & Safety Requirements

• Fit Test – Required for mask fitting (specific to Iowa, but generally applicable)
• Trinity 10 Panel Drug Screen – Completed within 30 days of start, must include specific substances (no THC)
• COVID-19 Vaccine – Proof of vaccine or approved exemption/declination (declination not accepted in CA or IL)
• MMR – Proof of 2 vaccinations or IgG titer lab results
• Hepatitis B – Must have started vaccination series prior to start; titers not accepted
• Influenza Vaccine – Proof of vaccine, declination, or exemption (declination not accepted in CA or IL) for current flu season
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – TB testing with chest x-ray if positive, per regional requirements
• Varicella – Proof of vaccination or IgG titer lab results
• Physical/Health Statement – Documentation from exam within last year confirming fitness to work

Professional & Compliance Requirements

• Trinity Code of Conduct Form – Signed form required
• Trinity I9 – Standard employment eligibility verification
• Confidentiality & Security Agreement – Signed agreement regarding patient and proprietary information
• Integrity and Audit Services Form – Signed prior to start
• Specialty Specific Competency Testing – Copy of specialty exam completed within the last year
• Conflict of Interest Disclosure Form – Signed form required
• Criminal Background Check – Federal, nationwide, and county checks covering last 7 years, including SSN verification, dated within 30 days of start
• Education Verification – Highest level of academic education verified by third party screening
• EPLS (Excluded Parties List System) Check – Dated within past 30 days
• GSA/OIG Exclusion Check – Dated within past 30 days
• Joint Commission CORE Exam – Completed within the last year
• NSO (National Sex Offender) Check – Dated within 30 days of start, including aliases and statewide checks where applicable
• System Access Form – Must be completed and uploaded one week before start
• Skills Checklist – Provided by agency for clinical contractors

This position requires thorough credentialing, health screenings, and compliance documentation prior to onboarding. The shift schedule is Monday through Thursday afternoons/evenings, and the contract duration is approximately 13 weeks.
