Job Description
- Provide skilled home health therapy visits (6-8 visits per day, minimum 6 points per day)
- Patient population includes medical/surgical, cardiac, orthopedic (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson’s, failure to thrive, respiratory patients, neurological, tracheostomy, trauma, amputees (AKA, BKA), and ADLs
- Home skilled visits under Medicare, Medicare Advantage, and Commercial insurance
- 1-2 weekends per contract
- Self-scheduled visits and hours based on HomeCare office needs
- Use of laptop, MiFi for internet, and portable printers provided
- Documentation via BayCare Forms, BayCare Scheduling, and Ace Timesheet system
Required Skills and Experience
- Home health experience required
- OASIS training preferred
- Skills: Assessment, Cardiopulmonary/Circulatory, Documentation, Equipment and Devices (gait belt, rolling walker, sit-to-stand lift, sliding boards, collars, standard walker, supplemental oxygen, upper extremity prosthetics, wheelchair assessment), Orthopedics, Neurology, Home Exercise Program, Medications, Patient and family education, Recognition of signs & symptoms of physical, sexual, psychological abuse, Safety and injury prevention, Billing
- Minimum 1 year of experience required
- Must have valid driver’s license and reliable car for home visits
- COVID-19 vaccination required or exemption on file
- Flu shot required or declination on file; mask wearing mandatory at all times while at work
Dress Code
- Any solid color scrubs
Certifications Required
- BLS (Basic Life Support), AHA or RQI accepted
- State licensure valid for Florida
- Additional certifications may be required depending on role (e.g., ACLS, PALS, NRP, EFM, NIH Stroke Certification, CPI for Behavioral Health, etc.)
Competency Exams
- Medication/Pharmacology competency exam (score 80%+)
- Dysrhythmia/EKG competency exam for RNs in certain units (score 100% on lethal arrhythmia section)
- Color Blind Testing (Relias) prior to start
- Other competency exams as applicable to profession
Health and Safety Requirements
- Physical exam within 6 months of start
- Negative TB test or chest X-ray within 10 weeks of start (or within 1 year if prior positive PPD)
- Annual TB questionnaire completion
- Immunizations or titers for Measles (Rubeola), Mumps, Rubella, Varicella, Hepatitis B (titer, series, or declination)
- Standard 9-panel drug test within 30 days of start (instant tests not accepted)
- Seasonal flu vaccination or declination with mask requirement
- COVID-19 vaccination or approved exemption
- Respirator medical clearance annually if respirator use is required
Background and Compliance Requirements
- Background check within 1 year including 7 years of residence/work/school history, SS trace, county searches, nationwide sex offender, education verification
- EPLS/GSA/SAM and HHS/OIG checks required
- Foreign equivalency education evaluation if education completed outside USA or Puerto Rico
- Education verification for all healthcare professionals
- Florida AHCA Level II fingerprinting clearance required prior to start (Livescan electronic fingerprinting)
- References: 2 references from past 2 years required (not for non-clinical)
- Employment application and skills checklist required
- BayCare Health MSP privacy policy, affidavit, demographic information sheet required for clinical candidates
- Photo for badge required prior to start
- Personal or auto insurance required for home health positions (must be current throughout assignment)
- BayCare dress code and tobacco policy compliance
- Completion of BayCare Online Learning Center modules prior to start (4-8 hours, deadline Thursday before start)
- Fit mask acknowledgement required
- EMR signature request form required for home health candidates
This position is ideal for experienced home health therapy professionals who are comfortable with self-scheduling, have strong clinical skills across a broad patient population, and meet all licensure, certification, and compliance requirements for Florida home health care.
