Physical Therapist (PT) - Temporary Position

Location: Fort Dodge, Iowa
Contract Length: 13 weeks

Job Description

• Work as part of an interdisciplinary team providing professional and clinical care to patients.
• May be required to float within the same discipline across local campuses.
• Home health experience is a plus.
• Experience working with all age groups is required.
• Must have at least 1.5 years of PT experience.

Work Hours & Requirements

• Billable hours: 36-40 hours per week.
• Required to work 2 of 3 major winter holidays (Christmas, New Year's, or Thanksgiving).
• Limited time off allowed between 11/26/25-11/30/25 and 12/23/25-1/2/26 for all travelers regardless of holidays worked.

Certification & Licensing

• BLS certification required.
• Must hold or be willing to obtain an Iowa state PT license. Non-Iowa licensed candidates will be considered if willing/able to apply for Iowa licensure.

Submission Requirements

• Work history/resume.
• Skills checklist completed within 1 year of submission.
• License verification.
• Copy of required certifications.
• One supervisory reference required at time of submission.
• Proof of taxable address (one of the following documents):

  • Driver’s License
  • Current Utility Bill
  • Lease/Rental Agreement
  • Mortgage Statement/Bill
  • Homeowner’s or Renter’s Insurance Statement/Bill
  • Voter Registration
  • Bank Statement
  • Auto Insurance
  • Tax Return

• Electronic Medical Record system used: EPIC.
• Call back and holiday work may have additional pay multipliers (not detailed here).
