Registered Nurse Travel/Contract Position

Location: Lebanon, Pennsylvania
Facility: WellSpan Good Samaritan Hospital


Job Details

  • Weekend Shifts: As needed
  • Call Requirements: None
  • Holiday Shifts: As needed
  • Floating: May be asked to float to all Medical-Surgical floors
  • Patient Population: May care for COVID-19 patients
  • Facility Policy: Tobacco and fragrance-free environment

Required Documentation & Compliance

Background Checks

  • 7-year criminal background check within 30 days of start, updated annually
  • EPLS/GSA/SAM check within 30 days of start
  • HHS/OIG check within 30 days of start
  • PA FBI Fingerprinting within 1 year of start; results must be on file within 45 days of assignment start
  • Pennsylvania State Police PATCH search within 1 year of start (must be finalized prior to start)
  • Pennsylvania Child Abuse Search within 5 years of start; results must be on file within 45 days of assignment start

Certifications

  • BLS (Basic Life Support) required
  • ACLS, PALS, NRP, TNCC, NIH Stroke Certification as applicable (refer to unit/job description)
  • State License primary source verification required
  • NURSYS verification for RN, LPN/LVN, AP
  • Acknowledgement of Pass/Fail Testing required for all RNs except OR and Home Health

Competency Exams

  • Medication/Pharmacology competency exam as applicable
  • Dysrhythmia/EKG competency exam (pass/fail) required for all RNs except OR and Home Health

Health Documents

  • Physical exam within 90 days of start, signed by MD, DO, PA, or NPT
  • Tetanus/Diphtheria/Tdap vaccine within 10 years of start
  • Negative TB test (QGold or TSpot) within 90 days of start; PPD not accepted
  • Immunizations or positive titers required for:
    • Rubeola (Measles) – 2 doses or positive titer
    • Mumps – 2 doses or positive titer
    • Rubella – 2 doses or positive titer
    • Hepatitis B – positive titer or full vaccine series (3 doses) plus declination form if applicable
    • Varicella – 2 doses or positive titer
  • Drug testing: Negative 9-panel within 1 year (Aya requirement only)
  • Seasonal Flu vaccination or valid medical/religious exemption required by October 31 annually
  • COVID-19 vaccine, medical/religious exemption, or declination accepted
  • Respiratory Safety clearance within 1 year of start, renewed annually

Facility Specific Documents

  • Passport-style photo for badge (professional appearance, solid light background)
  • Medical Statement Non-Employee Ability to Work form (preferred within 90 days of start)
  • Review and sign:
    • Commitment to Health & Wellness
    • Code of Conduct
    • Confidentiality Agreement
    • Non-Employee Affidavit
  • I9 Attestation and Documentation Attestation (agency to complete)
  • Completion of Learning Management System (LMS) modules prior to start (PC/laptop only)
  • New Hire Orientation required by end of first day

Additional Requirements

  • Two professional references within 3 years
  • Most recent resume or employment application
  • Annual skills checklist
  • Performance evaluations completed onsite by unit managers during assignment

Notes

  • Candidate may be asked to float to various medical-surgical floors
  • Facility is tobacco and fragrance free; candidates should be notified prior to submission
  • Candidate may care for COVID-19 patients
  • Background checks and clearances must be finalized timely to avoid removal from assignment
