Registered Nurse - Home Health (Travel/Contract)

Location: West Bridgewater, Massachusetts
Contract Length: 13 weeks


Job Description

  • Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
  • Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Average patient load: 5-8 patients per day.
  • Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
  • Must be willing to travel up to 75 miles from home clinic, averaging about 250 miles per week.
  • Schedule: Monday to Friday, 8 AM to 5 PM, with weekend rotation approximately every 4-6 weeks.
  • On-call requirements vary; discuss during interview.
  • Dress code: Scrubs.
  • Charting system: Proficiency required in HomeCare HomeBase (HCHB) within the last 6 months; retraining required if not current. RNs must also be proficient in OASIS.
  • Local candidates accepted at the same rate.
  • Pending licenses not accepted; must have license in hand.
  • Some positions may have a Baylor Shift schedule (4x10s, Friday-Monday), but most are standard 5x8s Monday-Friday.

Requirements

  • Specialty: Registered Nurse - Home Health
  • Experience: Minimum 1 full year of recent Adult Home Health experience OR 1 full year Hospice experience (whichever is higher).
  • First-timers: Not accepted; must have recent specialty experience.
  • Certifications: Current CPR and BLS required.
  • Transportation: Must have reliable transportation, current driver's license, and agency-required liability insurance.

Productivity Expectations

  • Weekly productivity: 30 points (points assigned based on types of visits and activities).
  • Assigned 5-10 patients per day, equating to roughly 6-8 points daily.

Compliance & Documentation Requirements

  • Must complete and maintain all required background checks, including but not limited to:
    • 7-year standard background check with SS trace, county, nationwide, sex offender, and state-specific searches.
    • EPLS/GSA/SAM and HHS/OIG checks within 14 days of start and monthly thereafter.
    • Fingerprinting as required by state (varies by location).
  • Education verification required for nursing degree.
  • Must complete all required state-specific registry checks (e.g., Massachusetts Nurse Aide Registry).
  • Required health documents include:
    • Physical exam within 12 months of start.
    • Negative TB test or chest X-ray within 12 months.
    • Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
    • Drug testing (10-panel) per Aya standard.
    • Seasonal flu vaccination or documented declination.
    • COVID vaccination series or declination.
  • Must review and sign all required compliance documents, including but not limited to:
    • Code of Conduct, Confidentiality Forms, HIPAA and Privacy Policies, Compliance Training, and Home Health Scenarios.
  • Must complete OASIS E course if not proficient in HCHB within the last 6 months.

Additional Notes

  • Prior travel experience is not explicitly required for this position.
  • Weekend and on-call requirements vary by location and should be discussed during the interview.
  • Mileage reimbursement provided at IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).

This position is ideal for experienced Registered Nurses with a strong background in Adult Home Health nursing, who are comfortable with travel within a 75-mile radius and proficient in home health documentation systems.
