Job Description

Required Experience

• Varicella: Proof of 2 immunizations, positive titer, or declination with signed statement.
• Tdap: Proof of vaccination within past 10 years or declination with required signatures.
• Annual TB Questionnaire: Required within 6 months of start; if positive, evaluation and treatment proof required.
• Drug Screen: Negative 10-panel drug screen within 30 days prior to start.

• Core Competencies: Annual completion of HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives, etc.
• Badge Photo: Forward-facing, well-lit, neutral background photo in JPEG format.
• Contract Annual Education Packet: Required annually.
• Eye Exam & Color Vision: Ishihara Color Test and Snellen Vision test within 1 year prior to start.
