Shift: 8:00 AM - 4:30 PM with rotating weekend shifts

Job Description

Seeking an experienced Travel Care Manager RN for acute hospital case management. The role requires recent experience in both discharge planning and utilization review (UR) within a hospital setting. This is not a charge nurse, staff nurse, outpatient, home health, or hospice case manager position. Candidates must be able to start effectively by day 2, demonstrating strong knowledge of:

• Discharge planning processes for patient transitions to home health, SNF, LTAC, IRF
• Utilization review for medical necessity using MCG criteria
• Patient choice letters and IMMs documentation as required by Conditions of Participation

The facility uses Cerner as the EHR, MorCare for case management documentation, and CarePort for referral management.

Requirements

• Current BLS certification
• Minimum 3 years of acute hospital case management experience (preference for travel experience)
• Must have recent experience in discharge planning and utilization review in a hospital setting
• Permanent address must be more than 50 miles from the facility
