Job Opportunity: Registered Nurse - Home Health

Job Description

• Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits and periodic office meetings.
• Average patient load: 5-8 patients per day.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Schedule: Monday to Friday, 8 AM to 5 PM (some positions may have alternate schedules such as 4x10 shifts Friday-Monday).
• Patient visits occur both in care facilities and patients' homes.
• Must be willing to travel up to 75 miles from the home clinic.
• Use of electronic medical records systems: HomeCare HomeBase (HCHB), Oasis, and Outlook.
• On-call requirement: Every 4th weekend (varies, discuss during interview).
• Weekend requirements vary by location and profession.

Requirements

• Specialty: Registered Nurse in Adult Home Health or Hospice.
• Experience: Minimum 1 full year of recent specialty experience in Adult Home Health or Hospice (whichever is higher). First-time travelers are not accepted; recent specialty experience is mandatory.
• Certifications: Current CPR and BLS certification required.
• Must have a valid nursing license in hand (no pending licenses accepted).
• Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Proficiency with HomeCare HomeBase (HCHB) within the last 6 months is required; if proficiency is older, retraining will be required. RNs must also be proficient in Oasis.
• Prior travel experience is not explicitly required.

Additional Information

• Dress code: Scrubs.
• Productivity expectations: 25-30 weekly units.
• Mileage reimbursement at IRS standard rate; compensation is from patient home to patient home (commute to/from first and last patient is not billable).
• Locals are accepted at the same rate.

Compliance and Documentation Requirements

Candidates must complete and maintain the following prior to and during assignment:

• Valid Driver’s License/State ID.
• Background checks including 7-year history, sex offender search, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable.
• Fingerprinting as required by state regulations.
• Education verification (degree applicable to profession).
• Physical exam within 12 months confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months.
• Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
• Drug testing (10-panel).
• Seasonal flu vaccination or documented declination.
• COVID-19 vaccination series or declination.
• Completion of facility-specific compliance documents including confidentiality forms, HIPAA and privacy policies, compliance training, and home health/hospice core policy acknowledgments.
• References: Two references required for all placements.
• Work history verification: At least two most recent employers or last 7 years if only one employer.
