Hospice RN - Moab, Utah (Contract)

Job Summary

Seeking a dedicated and compassionate Hospice Registered Nurse (RN) to provide skilled nursing care and support to terminally ill patients and their families in accordance with the hospice plan of care and philosophy. The role involves working as part of an interdisciplinary team to deliver high-quality end-of-life care, primarily in patients' homes.

Key Responsibilities

  • Perform comprehensive assessments of patient needs and coordinate individualized care plans.
  • Provide pain and symptom management consistent with hospice care plans.
  • Educate and support patients and families on disease processes, self-care, end-of-life issues, and hospice philosophy.
  • Collaborate with physicians, social workers, chaplains, and volunteers.
  • Participate in on-call rotation (1-3 shifts per week, including 3 weekends during the contract).
  • Respond within 45 minutes to patient needs when on call.
  • Maintain accurate and timely documentation in compliance with policies.
  • Work holidays as required.

Qualifications

  • Current Registered Nurse (RN) license in Utah or compact state license.
  • Minimum of 3 years nursing experience, preferably in hospice, home health, or palliative care.
  • Strong clinical assessment skills.
  • Excellent communication, interpersonal, and organizational skills.
  • Compassionate demeanor with ability to support patients and families during difficult times.
  • Valid driver’s license and reliable transportation for home visits.
  • Current BLS certification.
  • Experience with Hospicesoft or Careficient EMR preferred.

Contract Details

  • Length: 13 weeks
  • On-call: 1-3 shifts per week, including weekends
  • Work holidays as scheduled
  • Level 4 Trauma facility
  • Blue scrubs preferred
