Job Opportunity: PT Outpatient - Travel/Contract

Location: Kingman, Arizona

Schedule

• 4 days a week, 10-hour shifts (7:00 AM - 5:30 PM)
• Tuesday off every week
• Facility closed on Saturday and Sunday
• Manager flexible with time off requests during the week, may require swapping days to ensure patient coverage

Experience Requirements

• Minimum 1 year of experience in specialty required
• Locals and first-time travelers accepted
• No prior travel experience required

Required Credentials & Certifications

• Government issued photo ID (Driver’s License/State ID)
• Background checks including:

  • 7-year work/living history search
  • EPLS/GSA/SAM verification (annual)
  • HHS/OIG verification (annual)
  • Education verification (transcript/diploma or NSCH verification)
• Certifications (as applicable):

  • ACLS (Advanced Cardiac Life Support)
  • BLS (Basic Life Support)
  • PALS (Pediatric Advanced Life Support)
  • NRP (Neonatal Resuscitation Procedure)
  • Tech Certification Verification (for allied professionals)
  • State License verification (primary source online)
• Competency exams as applicable (medication/pharmacology, specialty-specific)

Facility Specific Requirements

• Passport-style photo for badge
• Completion of facility-specific forms including:

  • Security and confidentiality agreements
  • Behavioral expectations
  • HIPAA and information security training
  • Agency attestation and candidate forms

Health & Immunization Requirements

• Physical exam within 1 year of start date
• Negative TB test or chest X-ray within 1 year
• Annual TB questionnaire
• Proof of immunity or vaccination for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (titer, declination, or vaccine series)
  • Varicella (titer, vaccine, or history of chickenpox)
• Negative 9-panel drug screen within 30 days of start
• Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)

Additional Requirements

• Two professional references within 3 years of start
• Annual skills checklist
