Job Description
- Position: Speech Language Pathologist (SLP)
- Setting: Pediatric outpatient
- Schedule: Monday - Thursday, 9:00 AM - 5:30 PM; Friday, 7:30 AM - 4:00 PM
- Workdays: 5 days per week, 8 hours per day
- Must live at least 25 miles from the facility
Required Qualifications
- Minimum 1 year of experience as a Speech Language Pathologist (highest stated requirement)
- Valid Speech Language Pathologist license in the state of Minnesota
- Criminal background check within one year of start date (including multiple checks such as sex offender, nationwide, education verification, etc.)
- Completion of Minnesota DHS Net Study Fingerprint process prior to start (no pending fingerprints allowed)
- Physical exam within 1 year of start
- Required vaccinations and health documentation including:
- Tdap vaccination (no religious declinations accepted)
- Negative TB test or chest X-ray per Minnesota state guidelines
- Proof of immunity or vaccination for Measles, Mumps, Rubella, Varicella
- Hepatitis B vaccination, positive titer, or declination
- Seasonal flu vaccination or documented declination (mask required if declined)
- Basic Life Support (BLS) certification (AHA BLS required)
- Education verification (third party verification required)
- Two professional references from recent employers within 2 years
- Skills checklist and work history verification as applicable
Additional Requirements & Notes
- Candidates must complete a Minnesota DHS Net Study Questionnaire immediately after signing the contract to initiate fingerprinting clearance.
- Fingerprinting must be completed within 2 weeks of registration; candidates may need to travel to a location offering livescan fingerprinting if not available locally.
- Background checks and health documentation must be current and meet facility and state requirements.
- Candidates will complete facility-specific education modules during orientation (approximately 16 hours).
- Dress code and confidentiality agreements must be adhered to.
- If called off by the facility, the traveler will still be paid for guaranteed contracted hours.
