Job Opportunity: Case Manager (Registered Nurse)

Location: Hillsboro, Oregon
Employment Type: Travel/Contract

Job Summary

  • Analyze patient medical records for quality assessment and discharge planning.
  • Provide documentation to insurance carriers for service authorization.
  • Coordinate discharge plans and orders for patients leaving the hospital.
  • Identify community service needs and make appropriate transition plans.
  • Requires creativity, problem-solving, multitasking, prioritization, and ability to work with minimal supervision.
  • Work schedule: 4x10 hour shifts (7:00 AM - 5:30 PM), variable schedule including weekend rotations (full weekend every third weekend).
  • Dress code: Business casual or scrubs.

Required Qualifications

Education

  • Bachelor or Master of Science in Nursing.

Experience

  • Minimum 1 year experience as an Inpatient Case Manager.
  • Preferred: 5 years hospital nursing experience in varied clinical settings.
  • Knowledge of insurance carriers, Medicare guidelines, community health, and social service resources.
  • Experience with complex discharges, care coordination, and arranging post-acute care services (DME, Home Health, Hospice, Skilled Nursing Facilities, Prior-Authorizations).

Licenses & Certifications

  • Current Registered Nurse license by the Oregon State Board of Nursing.
  • American Heart Association (AHA) Healthcare Provider Basic Life Support (BLS).
  • Advanced Cardiac Life Support (ACLS) - AHA provider only.
  • Pediatric Advanced Life Support (PALS) - AHA provider only (not required for ED OBS positions).

Other Required Certifications & Competencies

  • Orientation Safety Manual Acknowledgement (Bloodborne Pathogen, Tuberculosis, HIPAA training).
  • Medication Test/Pharmacology Competency Exam (annual, passing grade 85%+).
  • Competency Exam (annual, passing grade 85%+).

Health & Compliance Requirements

  • Physical exam within 1 year prior to start, confirming good physical and mental health and ability to perform job duties.
  • Fit mask testing onsite on first day; annual updates required.
  • Tetanus/Diphtheria/Tdap vaccine within 10 years or signed declination.
  • Negative TB test (Qgold/TSPOT) within 1 year; PPD not accepted. Annual TB questionnaire for past positives.
  • Immunity or vaccination documentation for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B (immune titer and 3 vaccines or signed declination)
    • Varicella
  • Drug testing: 10-panel including methamphetamines within 30 days prior to start (laboratory-based, SAMHSA certified).
  • Seasonal flu vaccination or signed declination during flu season (mask policy applies if declined).

Background & Verification Checks

  • 7-year criminal background check within 30 days of start; updated annually.
  • EPLS/GSA/SAM and HHS/OIG checks required 30 days prior to start and monthly updates during assignment.
  • Education verification for highest level of education.
  • International background check if residency outside the US in past 10 years.

Additional Requirements

  • At least 2 professional employment references required.
  • Skills checklist required.
  • Facility-specific documents including confidentiality agreements, vaccine declination forms, respiratory questionnaire, and photo for badge.
  • Full availability for compliance needs up to start date.
