Job Opportunity: Home Health RN Case Manager

Location: Barre, Vermont

Job Description

Experienced Home Health RN needed
OASIS experience required
Home health and case management experience required
Strong preference for an experienced traveler
Candidate must have no requested time off during the contract

Required Qualifications

Registered Nurse (RN) license appropriate for Vermont
Experience in home health nursing and case management
OASIS (Outcome and Assessment Information Set) experience

Additional Notes

Prior travel experience is strongly preferred but not explicitly required
Contract length is approximately 20 weeks
