Job Opportunity: Physical Therapist (PT) - Home Health (Travel/Contract)

Location: Griffin, Georgia

Contract Length: 13 weeks

Job Description

Provide physical therapy services in patients' homes (home visits).
Participate in periodic office meetings.
Average patient load: 5-6 patients per day.
Types of cases: Start of Care and Follow Ups.
Schedule: Monday to Friday, 8 AM to 5 PM (most shifts are standard 5x8s; some locations may have 4x10s Friday-Monday).
On-call: Yes, approximately once a month.
Travel: Must be willing to travel up to 75 miles from home clinic; daily travel mileage reimbursed at IRS standard rate (patient-to-patient travel compensated, commute to/from first and last patient is not billable).
EMR systems used: HomeCare HomeBase, Oasis, Outlook.
Potential for hire at the end of the contract.

Required Qualifications

Valid Physical Therapist license in Georgia.
Minimum 1 year of experience as a Physical Therapist within the last year.
Experience working with adult patients.
Experience in a Home Health setting.
Current BLS certification.
