Travel RN - Case Management

Job Description

Seeking an experienced Travel RN Case Manager for an acute hospital setting. The role involves discharge planning and utilization review (UR) within the hospital. Candidates must be able to start quickly and function effectively as an acute hospital case manager by the second day.

Key Responsibilities:

• Discharge planning to coordinate patient transitions to home health, SNF, LTAC, IRF
• Conduct utilization review for medical necessity using MCG criteria
• Provide patient choice letters and IMMs as required by Conditions of Participation
• Documentation using MorCare and referral management with CarePort
• Work with Cerner EHR system

Shift: 8:00 AM – 4:30 PM with rotating weekend shifts

Required Qualifications

• Valid RN license in Mississippi
• Associate of Science in Nursing / Associate Degree in Nursing
• Minimum 3 years of acute hospital case management experience, including case management experience within the last year
• Recent hospital utilization review experience, including utilization review using InterQual criteria
• Discharge planning experience in an acute hospital setting
• BLS certification

Additional Information

• Not a charge nurse, staff nurse, outpatient, home health, or hospice case manager role
• Facility uses Cerner EHR, MorCare for case management documentation, and CarePort for referrals

Experience Requirements

• Specialty years of experience: 3+ years in acute hospital case management
• Prior travel experience: Preferred but not explicitly required
